Acute decompensated heart failure (primary) 2. Hypertension, uncontrolled 3. Type 2 diabetes mellitus 4. Chronic kidney disease, Stage 3 5. Atrial fibrillation, persistent DISCHARGE MEDICATIONS: 1. Furosemide 40mg oral once daily 2. Lisinopril 10mg oral once daily 3. Carvedilol 12.5mg oral twice daily 4. Spironolactone 25mg oral once daily 5. Metformin 500mg oral twice daily (hold if Cr > 1.5) 6. Apixaban 5mg oral twice daily 7. Atorvastatin 40mg oral once daily at bedtime 8. KCl 20mEq oral once daily 9. Nitroglycerin 0.4mg SL PRN chest pain FOLLOW-UP: Cardiology in 1 week. PCP in 2 weeks. Repeat BMP in 3 days. Weight daily; return if weight increases >2 lbs overnight.
